These are the services your membership covers at participating clinics. You pay the agreed price at the clinic; Facil only charges the membership fee.

| Primary care follow-up visit | — | Must qualify as a follow-up under Facil Health records. |
| Specialist visit - standard rate | — | Office visit only. Related procedures and tests are paid separately. |
| Specialist visit with active add-on | — | The Specialist Add-On must be active on the service date. |
| Basic laboratory service or test | — | Only tests identified in the current Facil Health member fee schedule. |
| Annual basic laboratory panel | — | Once per 12-month membership period: CBC, CMP, TSH, lipid panel and U/A. A valid order may be required. |
| Pharmacy service visit | — | Charged per pharmacy visit. Medication charges are addressed separately. |
The price of each service is in your member portal once you enroll, and any participating clinic can confirm it before you are seen.
You pay that price directly to the clinic. Facil only charges the membership fee.
