Pay a monthly membership fee and get a fixed price, agreed in advance, at participating Facil clinics in Miami-Dade and Broward. See the exact amount in your member portal, or ask the clinic before you are seen.

This membership fee will not increase.
- Your membership is active the same day
- No waiting period, neither to enroll nor to use it.
- You pay the service price at the clinic
- Facil only charges the membership fee. It never charges for the medical service.
- No clinic is listed without a signed agreement
- A provider without a current agreement is not in the directory and does not apply a reduced fee.
- You can cancel at any time
- Cancelling stops the next renewal. You keep access until the end of the period you already paid for.

1. Choose your plan
Pick the option that fits your household. The membership fee does not increase.

2. Get your membership card
Digital right away, physical if you ask for one. Your access starts the same day.

3. Show it at the clinic
You pay the published price for the service, directly to the clinic.
Your membership card, the same day
The digital one works from the moment you enroll. It carries the provider directory address printed on it, as § 636.226 requires, so the front desk can check that it is a participating clinic.
What your membership includes
You pay a fixed price at the clinic, agreed in advance. Facil only charges the membership fee.

| Annual basic laboratory panel | — | Once per 12-month membership period: CBC, CMP, TSH, lipid panel and U/A. A valid order may be required. |
| Pharmacy service visit | — | Charged per pharmacy visit. Medication charges are addressed separately. |
Why you can trust this

Not a percentage to work out at the counter: each service has an amount agreed with the clinic, and you see it in your portal.

What this plan does not include
Medical Services Membership · § 636.212
- Hospital care and specialists outside the participating network.
- Emergency room visits and hospital admissions.
- Anything that is not listed in the fee schedule.
- It does not exempt you from any federal or state obligation.
